Hartlepool and East Durham Mind have a temporary part time vacancy for a Health Link Worker to work on the Step Forward Tees Valley Programme. Key element of the role is to work within the community to provide targeted support and interventions in order to make positive changes in relation to employability for local clients. These clients will typically have two or more complex needs, which could include physical or mental health barriers, housing or substance misuse.

Well Being Practitioner (East Durham)

30 - 37.5 Hours per week £20,000 -21,000pa pro rata - Fixed term until March 31st 2023. 

 Let’s Connect have a vacancy for a Wellbeing Practitioner to work on the Let’s Connect Project to work mainly in East Durham. Key elements of the role includes: 1-1 psychosocial support, supporting mutual aid groups/ psychoeducation group sessions, and initial asset based reviews with individuals. 

 This person will work with other partners and community groups to build awareness and referral pathways. Must have a minimum of one full years’ experience of working as a wellbeing practitioner (or counsellor) and effectively managing a diverse and demanding case load.
